Items needed for your appointment:
1. All radiology studies including any x-rays, MRI's or CT scans on a CD or regular films.
2. Past medical records including operative reports, radiology reports, injection histories, EMG and nerve conduction studies.
3. A list of all medications you are currently taking.
4. Your insurance cards and a photo ID. Please contact our billing office, MedNet, at (310)322-4278 if you have any questions regarding coverage by your insurance plan.
* If it is a Worker's Compensation case, we will need a written pre-authorization letter by your adjuster prior to making an appointment.
5. The names, phone numbers, and addresses of ALL physicians involved in your care.
